High School Athletic Participation forms ("Wake County Athletic Participation Form" and "Leesville Road High School Permission to Treat Form") are due to Ms. Susan Pridgen, the athletic trainer, by July 20th, 2009 for Fall Sports tryouts, October 26th, 2009 for Winter Sports tryouts, and February 1st, 2010 for Spring Sports tryouts. Forms can be downloaded by clicking here (PDF; file includes both forms) and should be turned into the main office over the summer. The sports medicine program at Leesville Road High School will be offering athletic physicals on the gym wing on May 21st, courtesy of Carolina Family Practice & Sports Medicine and Triangle Orthopedic Associates. Physicals will begin at 6:30 pm. The cost will be $20 per physical. Please make checks payable to “CFPSM.” Forms can be downloaded by clicking here (PDF) and should be turned in the night of physicals.
